Financial Aid Forms

The Financial Aid Office makes available many of the forms that are used in the financial aid application process at Glendale Community College. Because of the large number of forms used, and that many forms are year-specific, we have divided our forms into separate web pages. To determine which form you need, select the appropriate link below.

In order to view or print any of the forms you will need to have the Adobe Acrobat Reader installed on your computer. Select this link to download the FREE Adobe Acrobat Reader, or click on the image to the right.


BOG FEE WAIVER - Summer 2008 thru Spring 2009	If you want to apply for the BOARD OF GOVERNORS (BOG) FEE WAIVER for the Summer 2008 thru Spring 2009 semesters, you can download the application by CLICKING HERE.

Federal financial aid- Fall 2008 thru Summer 2009	If you want to apply for FEDERAL FINANCIAL AID you need to complete a 2008-2009 Free Application for Federal Student Aid (FAFSA). The FAFSA is available at the Financial Aid Office, or you can complete a FAFSA online at the FAFSA on the Web website.
FORMS	If you have already completed your 2008-2009 FAFSA and need additional forms (as requested by the Financial Aid Office) CLICK HERE to see a list of the forms you can download.

Financial aid forms can be submitted in the following ways:

You can MAIL your completed forms to the following address:

You can SUBMIT your completed forms IN PERSON by coming to the Financial Aid Office during our regular business hours:
Mondays from 9am to 4pm
Tuesdays from 9am to 7pm
Wednesdays from 9am to 7pm
Thursdays from 9am to 4pm
Fridays from 9am to 1pm
(Above schedule is subject to change)

You can FAX your completed forms to the Financial Aid Office at:
818-547-4469
NOTE: Personal income tax returns, W-2s, and other tax related papers may be faxed to our office. Other documents, including Driver Licenses, Social Security Cards, INS/DHS documents, passports, high school diplomas, and all transcripts CANNOT be faxed in. These items must be brought to the Financial Aid Office where we will make copies of the required items.

You can SUBMIT your completed forms by placing them in the white DROP BOX, located outside our front entrance doors.
