NOMINATION FORM

Have a local business or resident publicly honored by the Character & Ethics Project.

Nominate businesses or employees who best exemplify the “Guiding Principles” or Word of the Month of the Character and Ethics Project. Honorees will be presented with a certificate of appreciation acknowledging their commitment to good character and ethical behavior.

Recognition ceremonies are held quarterly on 3^rd Thursday of March 19,  June 17, Sept 16 and 2^nd Thursday of Dec 9, 2004 at the Glendale Central Library, 3:00 -5:00 PM. All nominations must be submitted at least 1 month prior to the ceremony.
